Paperwork and forms needed PRIOR to participation:
-Medical Information form
-Medical Clearance Form (njayf.org)
-Participant Form (page 2)
-Copy of final report card
-Proof of age. (Certified Copy of Birth Certificate with raised seal for all new registrants or, passport, Military ID, Baptismal Records) Parents must bring ORIGINAL birth certificate and a copy. The copy will be stamped and certified using a League stamp by a franchise official. The original birth certificate will be returned to the parent. Original birth certificate must be available at any time during the season for inspection by AYF Officials.
Failure to bring the information packet in its entirety will result in disqualification from the registered team, with no refund.
Registration fees:
-Include a $75 volunteer bond (per family). The bond is returned at the end of the season if a family fulfills two volunteer hours.
-In addition, each family is required to purchase $200 in raffle tickets. On the first day of practice (late July), each family must purchase these tickets (one book of 20 tickets at $10 a ticket). As families sell the tickets, they keep the money.
-Payment plans are available through the website.
